Below is an approximate list of materials that are required.

You will need...

For the sides of the framework
3.6m of 90mm x 45mm wood.

For the back of the framework - Plywood minimum 12mm thick
one piece 540mm wide by 387mm long
one piece 540mm wide by 412mm long
one piece 540mm wide by 587mm long

For the front of the framework - Plywood minimum 12mm thick
one piece 540mm wide by 500mm long
one piece 540mm wide by 450mm long
one piece 540mm wide by 500mm long

Also you will need three pieces of plastic sheet the same width and length as the front plywood pieces listed above.
The plastic sheet can be any thickness (for example 3mm). It is used to give the concrete a smooth finish.

For the top of the framework - The stop end
one piece of 150mm x 25mm board 540mm long

Reinforcing
10m of ribbed reinforcing rod 10mm diameter.

Concrete
You can make your own concrete using a mixture of cement, sand and gravel. For information on how to mix concrete go to www.buildeazy.com/newplans/eazylist/concrete_mix.html
Alternatively you can buy bags of concrete mix which only require the addition of water. I used around six 40kg bags of concrete mix. You will need more or less depending on the size of the footing hole.

Other bits and pieces
You will need screws to hold the framework together, tie wire to hold the reinforcing rod together, some wood (plywood or board) for the framework bracing. You will also need some blocks to pack the chair up and to clamp against the framework for strengthening.
